St. Mary of the Annunciation Baptism Registration Form

Baptisms are celebrated at 1pm in the afternoon of the second Sunday of every month. Parents who are presenting their first child for Baptism are required to attend a Baptism preparation session, which will be held on the Wednesday before the day of your child's Baptism at 6:30pm in the church. Please note the requirements for Godparents described below, under "Important Baptism Information" and "Godparent Information". 

**Important Baptism Information**
Baptism Preparation Session Information: Parents who are presenting their first child for Baptism are required to attend a Baptism preparation session, which will be held on the Wednesday before the day of your child's Baptism at 6:30pm in the church.
Please note: Expect a confirmation email within 10 days of submitting this Baptism registration form. If you do NOT receive a confirmation email regarding this Baptism after 10 days, please call us at (978) 774-7575. Registration for Baptism is NOT final until this Baptism Registration request form is confirmed. Please be aware no Baptisms are held during the Lenten season (Feb. 18th, 2024 through March 30th, 2024). The next Baptism to be held after the Lenten season will be April 14th, 2024. Baptisms will resume the second Sunday of each month.
Godparent Information: According to canon law of the Catholic Church, at least ONE of the Godparents must be a Roman Catholic who has received the sacrament of Confirmation. Only one Godparent is required. A second Godparent who is a baptized Christian may be chosen as well. If there are to be two Godparents, they must consist of one man and one woman.
